DMs with no experience trying too hard, "Save or die!".

Those words were said more than once by a DM, and we were in no actual danger. They just wanted to be suspenseful.

"Do you mean a d20, or a heroic sacrifice?"

Extra credit when one of the party (likely a summoned minion) is some regular geometric shape that can in theory be rolled.

Been there. Done that.
Almost made the player attack the NPC because my narration made him think that the NPC was attacking him, when in fact it was just some slapstick-level tiny prank-"injury".

I wish our friday gm would offer this.

Twice theres been some issue as two pcs in party can and do have the option to Counterspell.

But if its worded that it sounds like it doesnt, we either have to break his narrative, or let him get off spells scott free. Neithers fun for both sides.
